Our Services
At Hope Home Health Care, we provide person-centered, one-on-one support services for children and adults with intellectual and developmental disabilities (I/DD). Our approach is rooted in dignity, choice, and individualized care—supporting each person according to their unique needs, preferences, and personal goals.
We work closely with individuals, families, and service coordinators to ensure supports align with the individual’s ISP and long-term outcomes, while promoting independence, safety, and community inclusion.
Our services cover any combination of the following areas, based on need and type of disability:

Getting involved in the local community

Personal hygiene (Bathing, hair care, and other grooming tasks).

Keeping living space clean and organized through light housekeeping.

Nutrition: preparing meals and feeding assistance if needed.

Going shopping.
-
Transportation to a job or other appointments.

Medication management and medical equipment management.
-
Scheduling and attending medical appointments.
Supported Living
Our supported Living services provide individualized, one-on-one support to help individuals live safely and independently in their own homes while working toward personal goals.
Supported Living services may include any combination of the following, based on the individual’s needs and authorized supports:

Personal hygiene support (bathing, grooming, dressing)

Meal planning, food preparation, and nutrition support

Light housekeeping and maintaining a clean, safe living space

Grocery shopping and essential errands
-
Medication reminders and assistance with medication management, as authorized

Support with medical or adaptive equipment
-
Scheduling and attending medical appointments

Skill-building to promote independence in daily living
-
Support with communication and self-advocacy
